Shareholder Services Form Instructions
|
In order to add or change any of
the following privileges on your account: Checkwriting, Automatic
Asset Builder®, Dreyfus TeleTransfer, Wire Redemption, Telephone
Redemption or Telephone Exchange, simply complete the Shareholder
Services Form (PDF: 68 KB). Please note the following important
instructions:
- Complete Sections 1 through 3
with the current account information;
- Complete Section 5 if you are
adding or changing the Checkwriting Privilege;
Note: For joint
accounts, you may elect to have only one signature required on
your check. If you select this option, a Signature Guarantee
is required in Section 9.
- Complete Section 6 if you are
adding or changing the Automatic Asset Builder®
Privilege;
- Select the desired date and
indicate the dollar amount you would like to invest;
- Attach a pre-printed
voided check from your bank account (bank account owners
name(s) must be pre-printed on the check); and
- Complete Section 8 with the
signature(s) of all registered owner(s) of the Dreyfus account.
- Complete Section 7 if you are
adding or changing any of the TeleService Privilege(s); and
- Check the box to indicate that
you are updating the banking information (if applicable);
- Attach a pre-printed voided
check from your bank account if adding or changing banking
information for the Dreyfus TeleTransfer and Wire Redemption Privileges
only (bank account owners name(s) must be pre-printed
on the check); and
- Obtain a Signature Guarantee
in Section 9 if adding or changing the Dreyfus TeleTransfer or Wire
Redemption Privileges.
- Complete Section 8 with the signature(s)
of all registered owner(s).
For business organizations and trust
accounts where there are no authorized signer(s)/trustee(s) named
in the registration, or if two or more authorized signer(s)/trustee(s)
can act independently of one another, the Multi-Purpose
Certification Form (PDF: 45 KB) must also be completed,
unless there is a form on file authorizing the current signer(s).
Please note the following important instructions:
- Complete Sections 1 and 2 for
all registration types;
- Complete Section 3 for corporate
accounts or incorporated associations and obtain a Signature
Guarantee or a Corporate Seal;
and
- Complete Section 4 for trust
accounts, partnerships, estates and other entities and obtain
a Signature Guarantee.

Download

including management fees, charges, expenses, and share classes,
as applicable. Please read it carefully before you invest.
1. A Signature Guarantee is
a stamped or typewritten assurance from a financial institution
(guarantor) that a signature or endorsement is genuine. Please
check with your financial institution to be certain their guarantee
will meet the requirements stated below.
The Transfer Agent has adopted standards and procedures pursuant
to which Signature Guarantees in proper form generally will be accepted
from domestic banks, brokers, dealers, credit unions, national securities
exchanges, registered securities associations, clearing agencies
and savings associations. Signature Guarantees will also generally
be accepted from financial institutions participating in the New
York Stock Exchange Medallion Signature Program (MSP), the Securities
Transfer Agents Medallion Program (STAMP) and the Stock Exchanges
Medallion Program (SEMP) provided the transaction amount does not
exceed the guarantor's surety bond coverage. The Alpha Prefix obtained
for the Medallion Guarantee must cover the entire transaction request.
Notarization by a Notary Public is not an acceptable guarantee.
