In order to accomplish a smooth
change of the registration on your account(s), we would like to
remind you to:
- Read the entire Change
of Account Registration Form (PDF: 95 KB). Also read carefully
the "important information" section at the top of the
form.
- Re-apply for any privileges
you want on your new account(s), if they are offered by the fund(s)
in those account(s). You must complete Sections 7–10 to request
these privileges. The privileges on your current account(s)
will not automatically transfer to your new account.
- Have current account owner(s)
sign in Section 5, and obtain a Signature Guarantee in Section
6.
- Have all new owners sign in
Section 11.
